Required Athletic Forms (7th-12th Grade)
All athletes that plan on participating in athletics or band, incoming 7th through 12th graders, at Decatur ISD schools must have a yearly up to date physical and all online paperwork through Rank One completed before participation in athletics or band is allowed. At the bottom of the page are links to 4 different places. These links include:
- Rank One instructions, which highlights step by step on the process of filling out the online information,
- A link to the Rank One website to complete paperwork
- A page with information that needs to be emailed to Kamden Phillips or Brandon Pounds for new students to the district
- A link to the UIL pre participation physical paperwork if you choose to print one out for yourself
The 2024-25 school year's paperwork will be available to complete around the second week of July 2024 and physicals may be completed after April 1st to be eligible for the 2024-25 school year.
